Follow Silent_Wolf's recommendation and use the "Contact Us" link at the top of our site.  Forum posts will not initiate the review process for any bans.  The only way to appeal a ban is through the "Contact Us" link and open a new ticket.

He never submitted a ticket by using the contact us button as asked.  The forums or Facebook are not ways to ask to be unbanned. He was asked to submit a ticket by using the contact us button so the admins who review these could look at it but never did.

We get a lot of messages asking to be unbanned and if its not in the ticket system its easy to be missed.
