I think I have been through all that you might experience. If you need help then all you need to do is ask. I'm full of advice whether it's any good or not.
Do you have a creative sound blaster x-fi sound card? Since you had problems you might need to uninstall the driver from the device manager and get latest update from the website.
Run as administrator. You can check mark the 2 "run as administrator" in the properties of the launcher.
The last 2 times I did a graphics update it messed up my sound driver. I do not understand why there can be a correlation, but obviously it can happen.
Go to control panel and check over the properties first and then go to configuration and set up the recorder. I know you are familiar with all this stuff, but it doesn't hurt to remind a person.
Sorry if I ramble on.

Wesley's advice is pretty sound. If that doesn't work, I'm a software engineer and would be glad to see if I can help. I work from home every day except Wednesdays so just call my cell anytime, 276.494.9623.
Vista has a much-much improved implementation of sound drivers, but unfortunately it's also more complex. At the end of the day, Microsoft just isn't Apple. :/
